A skilled and compassionate Registered Nurse is needed for a full-time contract opportunity in the home health setting. This role is ideal for an experienced RN who excels in independent, patient-focused care and can manage the unique challenges of field-based nursing. As a valued professional, you will play a critical part in assessing and caring for patients in their homes while promoting high-quality, compliant healthcare.

Schedule and Key Details:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• 2532 points per week (typically 35 patient visits daily)
• Field-basedtravel to patient homes (mileage reimbursement provided)
• Utilizes Home Care Home Base (HCHB) for documentation
• OASIS assessment tool

Qualifications:

• Active RN license (state of practice) and in good standing
• Minimum 1 year recent home health experience (field experience preferred)
• Proficient with Home Care Home Base (HCHB)
• OASIS experience required
• Reliable transportation, valid drivers license, auto insurance
• Excellent clinical assessment, critical thinking, and communication skills
• Ability to independently manage a field-based schedule

Preferred Experience:

• BSN
• Direct experience with 2532 points/week caseload
• Familiarity with Medicare and home health compliance

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing including wound care, medication administration, IV therapy, and chronic disease management
• Accurately document all care and communication in HCHB
• Educate patients and families on care plans and disease management
• Coordinate with therapists and other healthcare professionals for integrated care
• Participate in case conferences, staff meetings, and educational sessions

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
