Provide compassionate, high-quality nursing care directly in patients homes as a Home Health Registered Nurse. This contract role is ideal for skilled RNs who excel in independent, field-based care, are adept at patient communication, and are dedicated to promoting optimal outcomes through individualized treatment plans.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, promptly identify any status changes, and communicate effectively with the care team and physicians.
• Educate patients and their families regarding treatment plans, medications, and preventive care.
• Maintain timely, detailed documentation within Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated care.
• Participate in case conferences and staff meetings; engage in ongoing professional development.
• Adhere to all regulatory and agency standards for quality care and documentation.

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent Home Health (field) nursing experience.
• Proficient in Home Care Home Base (HCHB) and OASIS tool.
• Strong clinical assessment and critical thinking skills.
• Dependable transportation, a valid drivers license, and auto insurance.
• Excellent organizational, time management, and communication skills.
• BSN and experience managing a 2532 point caseload per week preferred.
• Understanding of Medicare and home health compliance a plus.

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
