Deliver high-quality nursing care in a patients home environment with this dynamic Home Health Registered Nurse opportunity. This field-based, full-time contract role features a supportive scheduleMonday through Friday, 8:00 AM to 5:00 PMserving an assigned caseload of 2532 points weekly (typically 35 patient visits per day) over a 13-week period.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract duration

Responsibilities:

• Conduct OASIS-compliant, comprehensive patient assessments and develop individualized care plans.
• Deliver direct skilled nursing care such as wound and medication management, IV therapy, and chronic disease support.
• Carefully evaluate and document patient progress using the Home Care Home Base (HCHB) system.
• Communicate any status changes promptly to physicians and interdisciplinary teams.
• Educate patients and families on health conditions, medication compliance, and preventative care strategies.
• Collaborate with physical and occupational therapists, medical social workers, and home health aides for holistic care.
• Participate in care conferences, utilize ongoing education, and ensure regulatory compliance at all times.
• Travel daily to patient homes within your assigned territory (mileage reimbursement provided).

Qualifications:

• Current, active RN license in good standing in applicable state
• Minimum of 1 year of recent, field-based Home Health nursing experience
• Strong working knowledge of the Home Care Home Base (HCHB) documentation platform
• OASIS assessment experience
• Reliable transportation, valid drivers license, and auto insurance
• Excellent clinical judgment, time management, and communication skills
• BSN and prior experience managing 2532 points per week preferred
• Familiarity with Medicare/home health compliance a plus

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
