Job Description
Deliver compassionate, high-quality care in a field-based role as a Home Health Registered Nurse. This position allows you to provide essential nursing services directly to patients in their homes, fostering continuity and individualized care throughout a 13-week assignment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and establish tailored care plans.
- Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
- Continuously evaluate patient statusidentifying changes and coordinating with physicians and the interdisciplinary team.
- Educate patients and families on disease processes, medications, and preventive strategies.
- Maintain timely, accurate documentation in Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and HHAs to achieve the best clinical outcomes.
- Participate in case conferences, staff meetings, and continuing education.
Candidate Qualifications:
- Active RN license in good standing.
- At least one year of recent Home Health nursing experience (field experience preferred).
- Proficiency with HCHB electronic documentation system.
- Solid understanding and hands-on knowledge of OASIS.
- Reliable transportation with valid drivers license and insurance.
- Strong assessment, critical thinking, and communication skills.
- Able to manage a caseload of 2532 points weekly and work independently.
- BSN and previous experience with Medicare home health compliance preferred.
