Deliver compassionate, high-quality care in a field-based role as a Home Health Registered Nurse. This position allows you to provide essential nursing services directly to patients in their homes, fostering continuity and individualized care throughout a 13-week assignment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and establish tailored care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Continuously evaluate patient statusidentifying changes and coordinating with physicians and the interdisciplinary team.
• Educate patients and families on disease processes, medications, and preventive strategies.
• Maintain timely, accurate documentation in Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and HHAs to achieve the best clinical outcomes.
• Participate in case conferences, staff meetings, and continuing education.

Candidate Qualifications:

• Active RN license in good standing.
• At least one year of recent Home Health nursing experience (field experience preferred).
• Proficiency with HCHB electronic documentation system.
• Solid understanding and hands-on knowledge of OASIS.
• Reliable transportation with valid drivers license and insurance.
• Strong assessment, critical thinking, and communication skills.
• Able to manage a caseload of 2532 points weekly and work independently.
• BSN and previous experience with Medicare home health compliance preferred.
