An opportunity is available for a skilled and compassionate Registered Nurse specializing in Home Health to provide patient-centered care in a field-based setting. This contract role focuses on supporting individuals in their own homes, delivering continuity of care, and collaborating with interdisciplinary teams for comprehensive patient management.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and design individualized care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress and proactively communicate changes to physicians and care teams.
• Educate patients and families regarding disease processes, medications, and preventive care.
• Maintain timely and accurate electronic documentation in Home Care Home Base (HCHB).
• Coordinate care with therapists, social workers, and home health aides.
• Participate in case conferences, team meetings, and ongoing education.
• Uphold regulatory and agency documentation standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• At least 1 year of recent home health field experience.
• Strong proficiency with Home Care Home Base (HCHB) and OASIS.
• Demonstrated clinical assessment and critical thinking capabilities.
• Valid drivers license, reliable transportation, and auto insurance.
• Excellent communication, organizational, and time management skills.
• Ability to work independently and handle a flexible, field-based schedule.
• Preferred: BSN degree, experience managing 2532 points per week, and knowledge of Medicare compliance in home health.

$1,839 - $1,946/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
