Seeking a skilled and compassionate Registered Nurse to deliver high-quality care to patients in their homes. This contract role focuses on providing comprehensive, patient-centered nursing services, ensuring each individual receives personalized attention and support. Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Essential Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- OASIS assessment experience
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible schedule
- Strong clinical assessment, communication, and organizational skills
Preferred:
- Bachelors degree in Nursing (BSN)
- Experience managing a caseload of 2532 points per week
- Knowledge of Medicare and home health compliance standards
Key Responsibilities:
- Conduct OASIS assessments and create individualized care plans
- Provide skilled nursing care such as medication administration, wound care, IV therapy, and chronic disease management
- Monitor patient progress and promptly communicate changes to the healthcare team
- Educate patients and families regarding disease management, medications, and preventive care
- Maintain timely and accurate documentation in HCHB
- Collaborate with therapists, social workers, and home health aides to ensure coordinated care
- Participate in case conferences and continuing education
- Uphold all compliance and documentation standards
