Providing expert, compassionate care in the comfort of patients homes, this role offers an opportunity for a skilled Registered Nurse to make a meaningful impact through home health nursing. The position involves managing a caseload of patients, performing thorough assessments, developing personalized care plans, and collaborating closely with multidisciplinary teams to drive positive health outcomes.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs
- Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease oversight
- Monitor patient progress, identify changes, and communicate promptly with physicians and care teams
- Educate patients and families on treatment regimens, disease management, and preventive care strategies
- Maintain accurate and timely documentation utilizing Home Care Home Base (HCHB) electronic records
- Collaborate with therapists, social workers, and home health aides to coordinate holistic care
- Participate in case conferences, team meetings, and continuing education activities
- Comply with all regulatory and agency standards governing patient care and documentation
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum 1 year of recent home health nursing experience; field experience strongly preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments required
- Strong clinical assessment abilities and critical thinking skills
- Valid drivers license, reliable vehicle, and auto insurance for patient visits
- Excellent communication, organization, and ability to work independently in a field-based role
- BSN preferred
- Prior experience managing 2532 points per week and familiarity with Medicare/home health compliance a plus
Schedule & Contract Details:
- Full-time 13-week contract
- Monday through Friday, 8:00 AM5:00 PM
- Patient caseload averaging 2532 points weekly (approximately 35 home visits daily)
- Daily travel within assigned territory with mileage reimbursement
