Providing expert, compassionate care in the comfort of patients homes, this role offers an opportunity for a skilled Registered Nurse to make a meaningful impact through home health nursing. The position involves managing a caseload of patients, performing thorough assessments, developing personalized care plans, and collaborating closely with multidisciplinary teams to drive positive health outcomes.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs
  • Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease oversight
  • Monitor patient progress, identify changes, and communicate promptly with physicians and care teams
  • Educate patients and families on treatment regimens, disease management, and preventive care strategies
  • Maintain accurate and timely documentation utilizing Home Care Home Base (HCHB) electronic records
  • Collaborate with therapists, social workers, and home health aides to coordinate holistic care
  • Participate in case conferences, team meetings, and continuing education activities
  • Comply with all regulatory and agency standards governing patient care and documentation

Qualifications:

  • Active RN license in good standing for the state of practice
  • Minimum 1 year of recent home health nursing experience; field experience strongly preferred
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments required
  • Strong clinical assessment abilities and critical thinking skills
  • Valid drivers license, reliable vehicle, and auto insurance for patient visits
  • Excellent communication, organization, and ability to work independently in a field-based role
  • BSN preferred
  • Prior experience managing 2532 points per week and familiarity with Medicare/home health compliance a plus

Schedule & Contract Details:

  • Full-time 13-week contract
  • Monday through Friday, 8:00 AM5:00 PM
  • Patient caseload averaging 2532 points weekly (approximately 35 home visits daily)
  • Daily travel within assigned territory with mileage reimbursement
