Deliver high-quality patient care while enjoying the autonomy and flexibility of field-based nursing as a Home Health Registered Nurse (RN). In this crucial role, you'll bring compassion and clinical expertise directly to patients homes, playing a vital part in their health journeys through individualized care planning, skilled nursing interventions, and collaborative teamwork.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and create patient-specific care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patients progress, notice changes in condition, and coordinate with physicians and the care team as needed.
• Educate patients and their families on disease management, medications, and preventive strategies.
• Maintain thorough, timely documentation using Home Care Home Base (HCHB).
• Work collaboratively with PTs, OTs, medical social workers, and home health aides to ensure unified care.
• Participate in case conferences, team meetings, and ongoing education.
• Follow all regulatory and agency standards for documentation and patient care.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• At least 1 year of recent home health nursing experience (field work preferred).
• Proficiency in Home Care Home Base (HCHB) and experience with OASIS.
• Strong assessment, organization, and communication abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Comfort working independently and efficiently managing a flexible field schedule.

Preferred:

• BSN degree.
• Experience handling 2532 points weekly.
• Familiarity with Medicare and home health compliance.
