Job Description
We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes. This position focuses on providing patient-centered, high-quality clinical care while maintaining seamless communication and detailed documentation. The right candidate is self-driven, highly organized, and confident in working independently across various home settings.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Travel: Daily visits to patient homes within an assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop customized care plans.
- Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate promptly with physicians and interdisciplinary teams.
- Educate patients and families on disease processes, treatment plans, and preventive care strategies.
- Accurately document care and assessments using Home Care Home Base (HCHB).
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate holistic patient care.
- Attend case conferences and staff meetings as required.
- Remain compliant with regulatory and agency standards for documentation and care delivery.
Required Qualifications:
- Active RN license in good standing for the state of practice
- At least one year of recent home health field nursing experience
- Proficiency with Home Care Home Base (HCHB) documentation system
- Experience with OASIS assessments
- Reliable transportation, valid driver's license, and auto insurance
- Strong clinical assessment, time management, and organizational skills
Preferred Credentials:
- BSN degree
- Background managing caseloads between 2532 points per week
- Familiarity with Medicare and home health compliance standards
