Job Description

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes. This position focuses on providing patient-centered, high-quality clinical care while maintaining seamless communication and detailed documentation. The right candidate is self-driven, highly organized, and confident in working independently across various home settings.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (typically 35 patient visits per day)
  • Travel: Daily visits to patient homes within an assigned territory (mileage reimbursement provided)

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop customized care plans.
  • Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and communicate promptly with physicians and interdisciplinary teams.
  • Educate patients and families on disease processes, treatment plans, and preventive care strategies.
  • Accurately document care and assessments using Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate holistic patient care.
  • Attend case conferences and staff meetings as required.
  • Remain compliant with regulatory and agency standards for documentation and care delivery.

Required Qualifications:

  • Active RN license in good standing for the state of practice
  • At least one year of recent home health field nursing experience
  • Proficiency with Home Care Home Base (HCHB) documentation system
  • Experience with OASIS assessments
  • Reliable transportation, valid driver's license, and auto insurance
  • Strong clinical assessment, time management, and organizational skills

Preferred Credentials:

  • BSN degree
  • Background managing caseloads between 2532 points per week
  • Familiarity with Medicare and home health compliance standards
