Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patients homes, focusing on quality, continuity, and patient-centered care. This field-based role requires a motivated, organized professional comfortable working independently while upholding high clinical standards and effective patient communication.
Responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and providing skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. You will evaluate patient progress, identify changes in condition, and coordinate with physicians and interdisciplinary teams including physical therapists, occupational therapists, social workers, and home health aides. Patient and family education on disease management and preventive care is essential. Accurate documentation using Home Care Home Base (HCHB) is required, along with compliance with all regulatory and agency standards.
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum 1 year recent home health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB)
- Experience with OASIS assessments
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with excellent communication, time management, and organizational abilities
Preferred:
- BSN degree
- Experience managing 2532 points per week caseload
- Knowledge of Medicare and home health compliance standards
Job Details:
- Contract Type: Full-time, 13-week contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (35 patient visits daily)
- Travel: Daily travel within assigned territory; mileage reimbursement provided
- Documentation: Home Care Home Base (HCHB)
