Qualifications & Experience:

• Current and unrestricted RN license
• Minimum of one year of recent experience in home health nursing
• Strong clinical assessment and critical thinking skills
• Familiarity with electronic documentation systems
• Ability to work autonomously and manage a diverse patient caseload
• Reliable transportation and a valid drivers license
• Excellent communication, organization, and time management abilities

Schedule/Hours:

• Flexible contract assignments available to accommodate your availability

Key Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Administer prescribed treatments, medications, and wound care per physician orders
• Educate patients and their families on disease management, medication administration, and home safety
• Monitor patient progress and collaborate with interdisciplinary teams to adjust care as needed
• Accurately document all care activities and patient responses in accordance with agency policies
• Serve as a resource for patients and families, providing emotional support and guidance

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
