Job Description
Deliver compassionate, patient-centered care as a Home Health Registered Nurse (RN) supporting individuals in their own homes. This role involves providing skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management. A successful candidate will be organized, self-motivated, and capable of managing a caseload independently while maintaining clinical excellence and open communication with patients and interdisciplinary teams.
Key Responsibilities:
- Conduct comprehensive OASIS assessments; develop and implement individualized care plans.
- Provide skilled nursing care tailored to patient needs.
- Monitor patient progress, identify changes, and communicate findings to care teams and physicians.
- Educate patients and families on treatment plans, medications, and disease management.
- Maintain timely, accurate documentation using Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and aides to ensure coordinated care.
- Participate in case conferences, staff meetings, and continuing education.
- Comply with regulatory and agency standards for care delivery and documentation.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum 1 year recent home health experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS required.
- Strong clinical assessment, critical thinking, and organizational skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently with a flexible, field-based schedule.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing 2532 points weekly caseload.
- Knowledge of Medicare and home health compliance standards.
Schedule & Details:
- Full-time, 13-week contract.
- Monday Friday, 8:00 AM 5:00 PM.
- Caseload expectation: 2532 points per week (35 patient visits daily).
- Field-based position with daily travel to patient homes; mileage reimbursed.
