Deliver skilled nursing care in patients homes as a Home Health Registered Nurse (RN), providing comprehensive assessments, individualized care plans, and patient education to support quality, patient-centered outcomes. This role requires clinical expertise, autonomy, and effective collaboration across interdisciplinary teams.

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify clinical changes promptly, and communicate effectively with physicians and care teams.
• Educate patients and families on disease management, treatment plans, and preventive care.
• Maintain accurate documentation within the Home Care Home Base (HCHB) system.
• Collaborate with therapists, social workers, and aides to ensure coordinated, quality care.
• Participate in case conferences, staff meetings, and continuing education programs.
• Comply with all regulatory and agency standards for patient care and documentation.

Qualifications:

• Active RN license in good standing with the state of Maryland.
• Minimum 1 year recent home health nursing experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment and critical thinking abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage an independent, flexible schedule.
• Excellent communication, organizational, and time management skills.

Preferred:

• BSN degree preferred.
• Experience managing 2532 points weekly.
• Knowledge of Medicare and home health compliance standards.

Job Details:

• Contract Type: Full-time, 13-week contract.
• Schedule: Monday Friday, 8:00 AM 5:00 PM.
• Caseload: Approximately 2532 points per week (35 patient visits daily).
• Field-based with daily travel to patient homes; mileage reimbursed.
