Job Description

An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver expert nursing care within patient residences, emphasizing quality, continuity, and patient-centered approaches. This fully field-based role requires independent clinical excellence, effective communication, and strong organizational skills to manage a caseload efficiently.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Administer skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes in condition, and communicate promptly with the interdisciplinary team and physicians.
  • Educate patients and their families on health conditions, treatments, medications, and preventive strategies.
  • Complete accurate documentation within the Home Care Home Base (HCHB) electronic system.
  • Coordinate with physical therapists, occupational therapists, social workers, and home health aides to ensure seamless patient care.
  • Participate in case conferences, staff meetings, and ongoing education to maintain clinical competencies.
  • Adhere to all regulatory and agency standards related to documentation and patient care.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum one year recent experience in Home Health nursing, with preference for field-based practice.
  • Proficiency with Home Care Home Base (HCHB) and experience using OASIS assessment tools.
  • Strong clinical assessment and critical thinking capabilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to manage a flexible, independent field-based schedule.
  • Excellent communication, time management, and organizational skills. Preferred:
  • Bachelor of Science in Nursing (BSN).
  • Experience managing caseloads of 2532 points weekly.
  • Knowledge of Medicare and home health compliance standards.

Schedule & Contract Details:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM 5:00 PM.
  • Caseload expectation: 2532 points per week (approximately 35 patient visits daily).
  • Daily travel within assigned territory with mileage reimbursement provided.
