Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract commitment
- 2532 points per week (typically 35 patient visits per day)
Qualifications:
- Active and unrestricted RN license in the state of practice
- Minimum of one year of recent home health nursing experience (field experience highly valued)
- Proficient with Home Care Home Base (HCHB) electronic documentation
- Demonstrated experience with OASIS assessments and care planning
- Strong critical thinking, organization, and communication skills
- Reliable transportation, valid drivers license, and up-to-date auto insurance
- Ability to work independently and manage a flexible schedule
- BSN preferred
- Understanding of Medicare and home health compliance standards is a plus
Responsibilities:
- Perform thorough OASIS assessments and establish individualized care plans
- Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
- Regularly monitor patient status, promptly communicate changes with the care team, and collaborate with interdisciplinary professionals
- Educate patients and their families, providing guidance on conditions, medications, and preventive care
- Maintain timely and accurate notes in Home Care Home Base (HCHB)
- Attend case conferences, staff meetings, and continuing education sessions as needed
- Ensure compliance with all regulatory requirements and agency policies
