Job Description
An experienced and compassionate Registered Nurse is needed to provide skilled home health care. This field-based opportunity centers on patient-focused nursing within patients homes, promoting quality outcomes and continuity of care. Youll work independently while collaborating with a supportive interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive OASIS assessments and craft individualized care plans.
- Deliver professional nursing carewound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient status, report changes to physicians, and adjust plans of care as needed.
- Educate patients and families regarding disease management, medications, and preventive strategies.
- Ensure timely, accurate documentation in Home Care Home Base (HCHB).
- Coordinate care collaboratively with therapists, medical social workers, and aides.
- Attend case conferences and ongoing professional development sessions.
- Maintain adherence to regulatory and agency standards.
Qualifications:
- Active, unencumbered RN license in the state of practice.
- At least 1 year of recent Home Health nursing experience (field-based preferred).
- Proficient in Home Care Home Base (HCHB) and OASIS assessments.
- Outstanding clinical assessment and critical thinking skills.
- Reliable transportation, current drivers license, and auto insurance.
- Ability to work independently, with excellent time management and communication.
- BSN and prior management of 2532 points/week preferred.
- Familiarity with Medicare and home health compliance standards a plus.
