An opportunity is available for an experienced and compassionate Registered Nurse to deliver home health services, ensuring the highest standards of care for patients in their own homes. This role requires a dedicated nursing professional who is self-driven, detail-oriented, and adept at working autonomously while maintaining strong communication across interdisciplinary teams.
Schedule and Setting:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract assignment
- Field-based: travel to patient homes within assigned territory (mileage reimbursement available)
- Expectation of 2532 points per week, typically involving 35 patient visits per day
Responsibilities:
- Complete comprehensive OASIS assessments and establish individualized patient care plans
- Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
- Track patient progress, identify changes in condition, and communicate efficiently with physicians and the care team
- Educate patients and their families on disease management, medication regimens, and healthy living strategies
- Maintain timely and accurate documentation using Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, and home health aides to coordinate care
- Participate in team meetings, case conferences, and ongoing education
- Uphold all regulatory and documentation standards
Qualifications:
- Active RN license in good standing
- Minimum 1 year of recent home health experience, with field experience preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Reliable transportation with valid drivers license and current auto insurance
- Strong assessment, communication, and organizational skills
- BSN preferred
- Experience managing a caseload of 2532 points weekly and familiarity with Medicare/home health compliance is a plus
