An opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled care to patients in their homes. This position focuses on delivering patient-centered nursing, comprehensive assessments, and collaborative care within a contract structure designed for motivated clinicians who enjoy independent fieldwork.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based visits to patient homes
- 13-week contract duration
- Caseload of 2532 points per week (typically 35 patient visits daily)
Key Responsibilities:
- Conduct detailed OASIS assessments and implement individualized care plans
- Administer skilled nursing interventions, including wound care, medication administration, IV therapies, and chronic disease management
- Monitor patient progress, promptly communicating changes to the interdisciplinary care team and physicians
- Educate patients and families about disease management, medications, and home safety
- Maintain timely, accurate documentation using Home Care Home Base (HCHB)
- Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care delivery
- Participate in case conferences, ongoing staff development, and adherence to agency and regulatory standards
Qualifications:
- Current RN license (state of practice) in good standing
- Minimum 1 year of recent field-based Home Health RN experience
- Proficiency in Home Care Home Base (HCHB) and strong OASIS knowledge required
- Strong assessment, clinical decision-making, and communication skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to manage a flexible field schedule independently
- BSN and experience managing 2532 points per week preferred
