Job Description
An opportunity is available for an experienced and compassionate Home Health Registered Nurse to deliver high-quality nursing care to patients in their homes. This contract position is designed for a self-motivated RN who thrives working independently while remaining committed to clinical excellence and patient-focused service.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- 13-week assignment
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Daily local travel to patient homes within an assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care such as medication administration, wound care, IV therapy, and chronic disease management.
- Evaluate patient progress, recognize changes, and collaborate promptly with care teams and physicians.
- Educate patients and families about conditions, treatment plans, medications, and preventive care.
- Maintain timely, accurate electronic documentation using Home Care Home Base (HCHB).
- Work closely with therapy staff, social workers, and home health aides for integrated care.
- Participate in case conferences, staff meetings, and ongoing professional education.
- Adhere to regulatory standards and agency protocols for documentation and care delivery.
Candidate Qualifications:
- Active RN license in good standing (state of practice).
- At least 1 year of recent home health nursing experience; field experience preferred.
- Proficient with Home Care Home Base (HCHB) and OASIS assessments.
- Strong clinical assessment, time management, and organizational skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to function independently in a field-based setting.
- Excellent interpersonal and communication skills.
- BSN and prior experience managing 2532 points per week preferred.
- Familiarity with Medicare and home health compliance standards desired.
