Job Description
Deliver skilled nursing care in a home-based setting as a Registered Nurse specializing in Home Health. This full-time, 13-week contract role provides an opportunity to apply your clinical expertise and compassionate approach while working independently to support patient-centered outcomes. Your daily visits, typically 3 to 5 per day, will focus on providing comprehensive assessments and individualized care plans that promote patient health and well-being.
Key Responsibilities:
- Perform thorough OASIS assessments and develop tailored care plans.
- Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate effectively with interdisciplinary care teams and physicians.
- Educate patients and caregivers on disease management, medications, and preventive care.
- Document care activities accurately and promptly using Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and home health aides to ensure seamless care coordination.
- Participate in case conferences, staff meetings, and continuous education.
- Comply with all regulatory and agency standards related to home health nursing.
Qualifications:
- Active and valid RN license in the state of practice.
- Minimum of 1 year recent home health experience (field experience preferred).
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Strong clinical assessment, critical thinking, communication, and organizational skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to manage a flexible field-based schedule autonomously.
- BSN preferred.
- Familiarity with Medicare and home health compliance standards is advantageous.
- Experience managing caseloads of 2532 points per week is a plus.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM.
