Job Description

An experienced and compassionate Registered Nurse (RN) with a background in home health care is sought to deliver skilled nursing services to patients in their residences within the Shawnee, KS area. This full-time, contract position spans 13 weeks and emphasizes patient-centered care, clinical excellence, and clear communication. The role requires a proactive RN who can work independently, managing a caseload of 2532 points weekly, typically involving 35 patient visits daily.

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes, and communicate with care teams and physicians.
• Educate patients and families on conditions, treatments, medications, and preventive measures.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members to ensure coordinated care.
• Participate in case conferences, meetings, and continuing education.
• Adhere to regulatory standards and agency policies.

Qualifications:

• Active RN license in good standing within the state of practice.
• Minimum of 1 year of recent home health experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment, critical thinking, and communication abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with a flexible, field-based schedule.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points weekly.
• Knowledge of Medicare and home health compliance standards.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM.
