Deliver compassionate nursing care and make a real impact in patients lives as a Home Health Registered Nurse. As an essential member of a collaborative care team, youll provide expert clinical care to patients in their homes, ensuring the highest standards for safety, dignity, and personalized support. This opportunity is a full-time, 13-week contract, offering a dynamic and rewarding field-based setting.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Requirements:
- Active RN license in good standing
- Minimum of 1 year of recent home health nursing experience (field experience strongly preferred)
- Demonstrated proficiency using the Home Care Home Base (HCHB) system
- Solid experience with OASIS assessments and documentation
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work autonomously and prioritize a flexible caseload (2532 points per week; 35 patient visits per day)
Preferred qualifications:
- Bachelors degree in Nursing (BSN)
- Familiarity with Medicare/home health compliance
- Experience managing a high-point home health caseload
Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans using OASIS
- Deliver skilled nursing care, including wound and medication management, IV therapy, and chronic disease education
- Evaluate patient progress, respond to changes, and communicate promptly with the care team and physicians
- Educate patients and caregivers on health management strategies and preventive care
- Ensure accurate, timely documentation in Home Care Home Base (HCHB)
- Collaborate closely with therapists and home health aides to support optimal patient outcomes
- Attend case conferences and participate in ongoing professional development
