A highly motivated and compassionate Registered Nurse is needed to deliver exceptional home health care to patients in a field-based setting. This contract opportunity focuses on providing skilled nursing to individuals in the comfort of their own homes, supporting both quality of life and clinical outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Complete comprehensive OASIS assessments and formulate individualized care plans.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication management, and chronic disease monitoring.
• Evaluate patient progress and communicate any changes in condition to the care team and physicians.
• Educate patients and families regarding diagnoses, treatment regimens, medications, and preventive health strategies.
• Ensure timely, accurate electronic documentation within Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members such as therapists, social workers, and aides for coordinated care.
• Attend case conferences, staff meetings, and continuing education sessions as needed.
• Adhere to agency policies, as well as regulatory and compliance standards.

Qualifications:

• Current, active RN license in the state of practice (good standing).
• At least 1 year of recent field-based home health RN experience required.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Reliable vehicle, valid drivers license, and current auto insurance.
• Strong assessment, independent decision-making, and communication skills.
• BSN and prior experience with 2532 points per week caseload preferred.
• Familiarity with Medicare and home health regulatory standards is advantageous.
