Seeking an experienced and compassionate Registered Nurse to deliver high-quality care in a home health setting. As the primary caregiver in the patients home, you will ensure clinical excellence, continuity of care, and patient-centered services. This field-based contract assignment lasts 13 weeks, offering a great opportunity for RNs who are skilled at working independently and enjoy making a direct impact on patient lives.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Expectations:

• Manage a caseload of 2532 points per week (typically 35 patient visits per day)
• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services: wound care, medication administration, IV therapy, chronic disease management
• Accurately assess patient progress and communicate changes to the healthcare team and physicians
• Educate patients and their families regarding disease management, medications, and care plans
• Complete timely documentation in the Home Care Home Base (HCHB) system
• Collaborate with interdisciplinary professionals to coordinate patient care
• Participate in staff meetings, case conferences, and required trainings
• Adhere to regulatory standards and agency policies

Minimum Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent home health nursing experience (field experience preferred)
• Proficiency in Home Care Home Base (HCHB) and OASIS required
• Strong clinical assessment and independent critical thinking skills
• Reliable transportation, valid drivers license, and current auto insurance
• Excellent communication, time management, and organizational abilities

Preferred:

• BSN degree
• Experience managing 2532 points per week
• Knowledge of Medicare and home health compliance regulations
