Job Description
Deliver compassionate, high-quality care as a Home Health Registered Nurse, supporting individuals in a field-based setting. This important contract position offers the autonomy to independently manage patient visits while providing clinical excellence and patient-centered support throughout the care continuum.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Responsibilities:
- Perform comprehensive patient assessments, utilizing the OASIS tool for detailed care planning
- Deliver skilled nursing care, including wound care, medication management, IV therapy, and chronic disease intervention
- Monitor patient progress, identify changes in condition, and promptly update interdisciplinary healthcare teams and physicians
- Provide ongoing education to patients and families regarding disease management, medication regimens, and preventive strategies
- Maintain thorough and timely electronic documentation using Home Care Home Base (HCHB)
- Collaborate with physical and occupational therapists, medical social workers, and home health aides to ensure holistic patient support
- Attend case conferences and staff meetings as needed, in addition to staying current through ongoing education
- Adhere to all applicable regulatory requirements and agency protocols for patient care delivery
Qualifications:
- Active, unencumbered RN license in state of practice
- Minimum one year of recent field-based home health nursing experience
- Proficient in Home Care Home Base (HCHB) and OASIS documentation
- Strong clinical assessment, planning, and organization skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN preferred
- Previous experience managing a caseload of 2532 points/week and knowledge of Medicare guidelines strongly desired
