Compassionate and skilled nurses are invited to take on a vital role delivering patient-centered care within a home health setting. This contract position is ideal for those with strong clinical skills and a commitment to supporting patients health and independence. Youll rely on your ability to work autonomously, communicate clearly, and maintain excellent standards of care as you visit patients in their homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Responsibilities:
- Perform comprehensive assessments using OASIS and develop individualized care plans.
- Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes, and collaborate with physicians and interdisciplinary team members to ensure optimal outcomes.
- Educate patients and families on care plans, medication, and disease management.
- Document care thoroughly and accurately in Home Care Home Base (HCHB).
- Participate in case conferences, staff meetings, and ongoing education.
- Ensure compliance with all regulatory and agency requirements.
- Travel daily between patient homes within your assigned area (mileage reimbursement provided).
Required Qualifications:
- Active RN license in good standing
- At least one year of recent home health experience
- Proficiency with Home Care Home Base (HCHB)
- Demonstrated OASIS experience
- Reliable transportation, active drivers license, and insurance
- Strong independent work habits and organizational skills
Preferred Qualifications:
- BSN
- Track record of managing 2532 points per week
- Familiarity with Medicare and compliance standards
