Job Description
Take your nursing career into the community as a Home Health Registered Nurse, where you'll make a tangible difference delivering skilled care to patients in their own homes. This full-time, 13-week contract offers the chance to provide compassionate, patient-centered clinical support as part of an interdisciplinary teamall while enjoying the independence of a field-based schedule.
Schedule and Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, visiting patient homes daily (mileage reimbursement provided)
- Caseload expectation: 2532 points per week (typically 35 patient visits per day)
- Utilize Home Care Home Base (HCHB) for electronic documentation
- Conduct assessments using OASIS
Qualifications:
- Active RN license (in good standing)
- Minimum 1 year of recent home health experience (field experience preferred)
- OASIS experience and proficiency with Home Care Home Base (HCHB) required
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work autonomously and manage a flexible schedule
- Excellent communication with patients, families, and clinical teams
Preferred Experience:
- BSN preferred
- Previous management of 2532 points weekly caseload
- Understanding of Medicare and home health compliance
Key Responsibilities:
- Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
- Complete comprehensive OASIS assessments and create tailored care plans
- Educate patients and families on care, prevention, and medications
- Collaborate with therapists, social workers, and home health aides for seamless care
- Maintain detailed, timely documentation and report changes in patient conditions
- Participate in team meetings, case conferences, and ongoing education
