Job Description

An excellent opportunity awaits for a skilled and compassionate Registered Nurse experienced in home health care. This contract position focuses on supporting patients in their own homes, ensuring high-quality, patient-centered nursing and coordinated multidisciplinary care.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Caseload: 2532 points weekly (approximately 35 patient visits per day)
  • Field-based, requiring daily travel across an assigned territory

Key Responsibilities:

  • Complete comprehensive OASIS assessments and craft detailed care plans for each patient.
  • Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management.
  • Evaluate patient health status, monitor progress, and promptly relay any significant changes to the broader care team and physicians.
  • Educate patients and their families about diagnoses, care plans, medications, and health promotion strategies.
  • Ensure accurate, timely documentation using Home Care Home Base (HCHB).
  • Work collaboratively with physical and occupational therapists, medical social workers, and home health aides to promote seamless patient care.
  • Participate in case conferences, staff meetings, and ongoing education sessions as needed.
  • Uphold all regulatory and best practice standards for home health delivery.

Qualifications:

  • Active, unencumbered RN license
  • At least one year of recent home health field experience
  • Proficiency in Home Care Home Base (HCHB) and OASIS required
  • Strong clinical judgment, time management, and organizational abilities
  • Ability to work independently in a field-based role
  • Reliable transportation, valid drivers license, and current auto insurance
  • BSN degree and Medicare home health knowledge preferred
