Key Responsibilities:
- Perform comprehensive OASIS assessments and develop personalized care plans.
- Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, detect changes promptly, and communicate effectively with physicians and interdisciplinary teams.
- Educate patients and families on treatment plans, medications, and preventive care.
- Maintain thorough and timely documentation using Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and aides to ensure seamless care delivery.
- Follow all regulatory standards and participate in continuing education.
Qualifications and Desired Experience:
- Active RN license in good standing within the state of practice.
- Minimum of 1 year recent home health experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool required.
- Strong clinical assessment, critical thinking, and communication skills.
- Reliable transportation with valid drivers license and insurance.
- BSN preferred.
- Knowledge of Medicare and home health compliance standards is a plus.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload typically includes 35 patient visits per day (2532 points weekly).
- Daily travel within the assigned territory with mileage reimbursement.
