An experienced Home Health Registered Nurse (RN) is sought to deliver skilled nursing care to patients within their homes, focusing on patient-centered and high-quality care. This full-time, 13-week contract opportunity involves managing a caseload of 2532 points per week, typically involving 35 patient visits daily, Monday through Friday from 8:00 AM to 5:00 PM.
Responsibilities include conducting comprehensive OASIS assessments, developing and managing individualized care plans, and providing a broad range of skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. You will evaluate patient progress, communicate changes to care teams and physicians, educate patients and families, and collaborate closely with interdisciplinary team members including therapists and social workers. Additionally, accurate documentation in Home Care Home Base (HCHB) and adherence to regulatory standards are essential. Daily travel within the assigned territory is required, with mileage reimbursement provided.
Qualifications and Desired Experience:
- Active RN License in good standing for the state of practice
- Minimum of 1 year recent home health experience with field experience preferred
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Experience with OASIS assessment tools
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible field-based schedule
- Excellent communication, organization, and time management skills
- BSN preferred
- Previous management of 2532 points weekly preferred
- Knowledge of Medicare and home health compliance standards is a plus
