We are seeking a skilled and compassionate Home Health Registered Nurse for a full-time, 13-week contract assignment. In this field-based position, youll have the autonomy to provide high-quality nursing care directly in patients homes, supporting their health, independence, and comfort.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Daily travel within your assigned territory (mileage reimbursement included)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized patient care plans.
- Deliver direct nursing servicesincluding wound care, IV therapy, medication administration, and chronic disease management.
- Closely monitor patient status, proactively communicating any changes to the care team and physicians.
- Educate patients and families on care procedures, disease management, and preventive health.
- Accurately and promptly complete electronic documentation using Home Care Home Base (HCHB).
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides.
- Participate actively in team meetings, case conferences, and ongoing education.
Qualifications:
- Active RN license in good standing within the state of practice.
- At least 1 year of recent home health nursing experience; field experience is preferred.
- Proficient with Home Care Home Base (HCHB) electronic documentation.
- Solid understanding and experience with OASIS.
- Reliable transportation, current drivers license, and valid auto insurance.
- Strong communication, organizational, and critical thinking skills.
- BSN and prior experience managing 2532 points/week preferred.
- Working knowledge of Medicare and home health compliance is advantageous.
