Deliver high-quality, patient-focused care as an experienced Home Health Registered Nurse. This full-time, 13-week contract offers the rewarding opportunity to make a real impact by supporting patients in their homes, ensuring their comfort and recovery. Youll work with a collaborative multidisciplinary team, using your expertise and independent judgment to promote positive outcomes.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Your role encompasses comprehensive patient assessments, development and management of individualized care plans, educating patients and families, and facilitating clear communication within the healthcare team. Youll visit 35 patients per day (2532 points per week), maintaining accurate electronic documentation via Home Care Home Base (HCHB) and using OASIS for assessments. Daily local travel is required, with mileage reimbursement provided.
Key Responsibilities:
- Perform thorough OASIS assessments and create patient-centric care plans
- Deliver skilled nursing services including medication administration, wound care, and IV therapy
- Monitor patient progress, identify changes, and communicate updates promptly to physicians and care teams
- Offer education to patients and families about their care, medications, and preventive strategies
- Complete timely, precise documentation in the electronic record system
- Collaborate with physical therapists, social workers, and home health aides to ensure coordinated care
- Participate in case conferences and ongoing training
Qualifications:
- Active RN license (in good standing) in the state of practice
- At least 1 year of recent Home Health RN experience (field work preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS assessments
- Strong assessment, communication, and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible field schedule
- BSN and familiarity with Medicare/home health compliance preferred
