Job Description
Experienced and compassionate Registered Nurses with home health expertise are invited to take on a rewarding role delivering patient-centered care in a field setting. This is a 13-week contract position providing skilled nursing to patients in their homes, with a schedule that supports work-life balance.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
What Youll Do:
- Conduct thorough OASIS assessments and develop personalized care plans
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease monitoring
- Monitor patient progress, promptly reporting changes in condition to physicians and the care team
- Educate patients and family members on treatments, medications, and self-care strategies
- Document assessment, interventions, and outcomes in Home Care Home Base (HCHB)
- Collaborate with other professionals, including therapists, social workers, and aides, to coordinate comprehensive care
- Attend case conferences, team meetings, and pursue ongoing education as required
- Ensure all patient care and documentation meet regulatory and agency standards
What Were Looking For:
- Current, active RN license in good standing in the state of practice
- Minimum of 1 year of recent home health nursing experience
- Working knowledge and proficiency with Home Care Home Base (HCHB) and OASIS
- Strong clinical, assessment, and decision-making skills
- Reliable vehicle, a valid drivers license, and up-to-date auto insurance
- Excellent communication, time management, and the ability to work both independently and as part of a multi-disciplinary team
Preferred:
- BSN degree
- Previous experience managing a caseload of 2532 points per week
- Understanding of Medicare and home health compliance
