If you are an experienced and compassionate Registered Nurse with a passion for providing quality, patient-centered care in a home setting, this opportunity could be an excellent fit. You'll work closely with individuals and their families to deliver expert nursing services and support patients on their path to wellness, serving as an essential part of their care journey.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Role Overview:
- 13-week full-time contract
- Field-based in patient homes
- Caseload of approximately 2532 points each week (typically 35 patient visits per day)
- Daily travel within assigned territory (mileage reimbursement provided)
- Utilization of Home Care Home Base (HCHB) for electronic documentation and OASIS for assessments
Key Responsibilities:
- Conduct comprehensive OASIS assessments and tailor individualized care plans
- Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, quickly address changes in condition, and communicate updates to healthcare teams
- Educate patients and their families about disease processes, treatments, and preventive care
- Accurately maintain documentation in compliance with regulatory and agency standards
- Coordinate care with therapists, social workers, and home health aides
- Engage in case conferences, staff meetings, and relevant continuing education
Qualifications:
- Current, active RN license (in good standing)
- At least 1 year of recent home health nursing experience (field experience preferred)
- Proficient in Home Care Home Base (HCHB) and OASIS
- Excellent assessment, communication, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN and Medicare/home health standards knowledge preferred
