Seeking an experienced and compassionate Registered Nurse to deliver skilled home health care within a field-based role. This opportunity offers a 13-week contract, providing autonomy, diverse clinical experiences, and the ability to support patients in their own homes while collaborating with an interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services such as wound care, medication management, IV infusions, and chronic disease monitoring.
• Monitor patient conditions, evaluate progress, and coordinate interventions with the healthcare team.
• Educate patients and caregivers about disease management, treatment regimens, medications, and wellness strategies.
• Maintain thorough, timely clinical documentation in the Home Care Home Base (HCHB) system.
• Work closely with therapy, social work, and aide staff to ensure comprehensive care.
• Participate in case conferences, staff meetings, and continued training opportunities.
• Ensure all care and documentation adheres to state, federal, and agency regulations.

Requirements:

• Active RN license in good standing
• At least one year of recent Home Health nursing experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) software
• OASIS assessment experience
• Strong clinical, assessment, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage a flexible field schedule

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN)
• Experience managing 2532 points/week
• Familiarity with Medicare and home health compliance

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
