A rewarding opportunity is available for a skilled and compassionate Home Health Registered Nurse to deliver high-quality, patient-centered care directly in patients homes. In this role, you will conduct comprehensive OASIS assessments, develop tailored care plans, and provide direct skilled nursing interventions, all while collaborating with an interdisciplinary team to ensure the best possible patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
What Youll Do:
- Conduct thorough OASIS assessments and create individualized plans of care.
- Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient progress, promptly addressing changes in condition and communicating with the healthcare team and physicians.
- Provide education and support to patients and families regarding diagnoses, medications, and treatment plans.
- Ensure timely, accurate documentation in Home Care Home Base (HCHB).
- Coordinate closely with collaboratorssuch as therapists, social workers, and home health aidesto provide holistic, coordinated care.
- Participate in team meetings, case conferences, and ongoing professional development.
- Uphold regulatory and agency standards in all aspects of patient care.
What Were Looking For:
- Active RN license (in good standing)
- Minimum one year of recent Home Health experience (field-based strongly desired)
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment, critical thinking, and communication skills
- Ability to independently manage a flexible, field-based schedule
- Reliable transportation, valid drivers license, and current auto insurance
- BSN and experience with Medicare compliance and managing 2532 weekly points preferred
