Job Description

Delivering compassionate, patient-centered care, this role offers experienced RNs the opportunity to make a meaningful difference in the lives of hospice patients and their families. As a Hospice RN, you will use your clinical expertise to coordinate and administer outstanding end-of-life care in home and residential settings. This contract position emphasizes support, dignity, and the highest quality of life for those facing terminal illness.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation may be required

Key Responsibilities:

  • Perform comprehensive nursing assessments and develop individualized hospice plans of care
  • Deliver skilled nursing interventions focused on pain and symptom management
  • Administer medications and treatments according to physician orders and established protocols
  • Monitor patient progress and promptly communicate changes to the medical director and care team
  • Educate patients and families on disease progression, comfort measures, and proper medication administration
  • Collaborate with interdisciplinary team membersincluding social workers, chaplains, and aidesto provide holistic support
  • Maintain clear and timely documentation in Home Care Home Base (HCHB)
  • Regular participation in case conferences, team meetings, and quality improvement initiatives
  • Ensure full compliance with hospice policies, procedures, and regulatory standards

Required Qualifications:

  • Active, unencumbered RN license in state of practice
  • At least 1 year of recent hospice nursing experience
  • Proficiency in Home Care Home Base (HCHB)
  • Strong clinical assessment, documentation, and communication skills
  • Deep understanding of end-of-life care and support principles
  • Reliable personal transportation, valid drivers license, and auto insurance
  • Ability to work independently in a field-based role with strong organizational skills

Preferred Qualifications:

  • BSN
  • Prior palliative or end-of-life care experience
  • Knowledge of Medicare hospice documentation
