Delivering compassionate, patient-centered care, this role offers experienced RNs the opportunity to make a meaningful difference in the lives of hospice patients and their families. As a Hospice RN, you will use your clinical expertise to coordinate and administer outstanding end-of-life care in home and residential settings. This contract position emphasizes support, dignity, and the highest quality of life for those facing terminal illness.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized hospice plans of care
- Deliver skilled nursing interventions focused on pain and symptom management
- Administer medications and treatments according to physician orders and established protocols
- Monitor patient progress and promptly communicate changes to the medical director and care team
- Educate patients and families on disease progression, comfort measures, and proper medication administration
- Collaborate with interdisciplinary team membersincluding social workers, chaplains, and aidesto provide holistic support
- Maintain clear and timely documentation in Home Care Home Base (HCHB)
- Regular participation in case conferences, team meetings, and quality improvement initiatives
- Ensure full compliance with hospice policies, procedures, and regulatory standards
Required Qualifications:
- Active, unencumbered RN license in state of practice
- At least 1 year of recent hospice nursing experience
- Proficiency in Home Care Home Base (HCHB)
- Strong clinical assessment, documentation, and communication skills
- Deep understanding of end-of-life care and support principles
- Reliable personal transportation, valid drivers license, and auto insurance
- Ability to work independently in a field-based role with strong organizational skills
Preferred Qualifications:
- BSN
- Prior palliative or end-of-life care experience
- Knowledge of Medicare hospice documentation
