A Hospice Registered Nurse (RN) role is available to provide compassionate, patient-centered end-of-life care in home, assisted living, and nursing home settings. This field-based position demands a dedicated professional focused on maintaining comfort, dignity, and quality of life for patients facing terminal illnesses, while also supporting their families during this critical time.

Contract Details:
- Full-time, 13-week contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM, with occasional weekend or on-call rotations
- Caseload: Approximately 45 patient visits daily
- Travel to patient locations within assigned service area; mileage reimbursement provided
Key Responsibilities:
- Conduct comprehensive nursing assessments and develop individualized hospice care plans
- Deliver direct skilled nursing focused on symptom management, pain control, and patient comfort
- Administer medications and treatments per physician orders and hospice protocols
- Monitor patient conditions and communicate updates promptly to the care team
- Educate patients, families, and caregivers on disease progression and comfort care strategies
- Collaborate with interdisciplinary teams including social workers, chaplains, CNAs, and volunteers
- Complete accurate documentation using the Home Care Home Base (HCHB) system
- Participate in case conferences, team meetings, and quality improvement initiatives
- Ensure compliance with hospice policies and regulatory guidelines
Qualifications & Experience:
- Active RN license in good standing for the state of practice
- Minimum of 1 year recent hospice nursing experience
- Proficiency with Home Care Home Base (HCHB) required
- Strong clinical assessment skills and excellent communication
- Reliable transportation with valid drivers license and insurance
- Ability to work independently managing a field-based schedule
- Compassionate, emotionally resilient
Preferred:
- BSN degree
- Experience in palliative or end-of-life care settings
- Familiarity with Medicare hospice regulations and documentation standards
