Bring your clinical expertise and passion for patient care to a vital role providing skilled nursing services in a home health setting. As a Home Health Registered Nurse, you will make a meaningful impact on patients lives by coordinating and delivering care in their homes, ensuring comfort and optimal outcomes.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Contract length: 13 weeks
• Caseload: 2532 points weekly (generally 35 home visits per day)
• Field-based role with daily travel within a defined territory (mileage reimbursement included)

Primary Responsibilities:

• Complete thorough OASIS assessments and develop individualized plans of care
• Deliver direct nursing services including medication administration, IV therapy, wound care, and disease management
• Monitor and evaluate patient progress, adjusting care plans and swiftly communicating changes to the care team
• Educate patients and their families on prescribed treatment, medications, chronic disease, and preventive care
• Ensure prompt, detailed documentation using Home Care Home Base (HCHB)
• Collaborate with multidisciplinary clinicians including therapists, social workers, and aides to deliver coordinated care
• Participate in case conferences, maintain regulatory compliance, and engage in ongoing training

Qualifications:

• Active RN license (state of practice)
• At least 1 year of recent home health (field) experience
• Skilled in Home Care Home Base (HCHB) and OASIS assessments
• Strong clinical judgment, communication, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently

Preferred:

• BSN degree
• Experience managing a points-based home health caseload
• Understanding of Medicare and home health regulations

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
