An exciting opportunity is available for an experienced and compassionate Registered Nurse with a strong background in home health care. In this field-based position, youll deliver skilled nursing services to patients in their residences, ensuring continuity, quality, and patient-centered care within a supportive interdisciplinary environment.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Field-based, providing care in patients homes
- Daily travel within your assigned territory (mileage reimbursement provided)
- Caseload: 2532 points per week (35 patient visits daily)
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care, such as medication administration, wound care, IV therapy, and chronic disease management
- Monitor patient conditions, track progress, and communicate with care teams and physicians
- Educate patients and their families on health conditions, treatments, medications, and preventive care
- Document care thoroughly using Home Care Home Base (HCHB)
- Coordinate with physical therapists, occupational therapists, social workers, and home health aides
- Participate in case conferences and maintain regulatory and agency standards
Requirements:
- Active, unencumbered RN license
- Minimum 1 year of recent home health (field) experience
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, and auto insurance
- Excellent critical thinking, assessment, organizational, and communication skills
- Ability to work independently and manage a flexible schedule
Preferred:
- BSN degree
- Prior experience meeting a 2532 point workload
- Familiarity with Medicare and compliance standards in home health

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
