Step into a vital role delivering high-quality nursing care directly to patients in the comfort of their homes. As a Home Health Registered Nurse, you will be responsible for supporting patients and their families through comprehensive assessments, care planning, and collaboration with interdisciplinary healthcare teams in a field-based setting.

Schedule & Assignment:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload: 2532 points per week (usually 35 visits per day)
• Location: Patient homes within an assigned territory; travel required (mileage reimbursed)

Primary Responsibilities:

• Complete thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient status, identify and report changes in condition, and update physicians and the care team as needed
• Provide clear education to patients and caregivers about disease processes, treatments, and medications
• Accurately document all care using Home Care Home Base (HCHB)
• Work collaboratively with therapists, social workers, and home health aides to ensure well-coordinated patient care
• Participate in case conferences, meetings, and ongoing education

Required Qualifications:

• Active and unrestricted RN license in the state of practice
• At least 1 year of recent, direct field-based Home Health experience
• Proficient with Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, current auto insurance
• Excellent assessment, time management, and communication skills
• Ability to function autonomously in a dynamic field environment

Preferred Background:

• Bachelor of Science in Nursing (BSN)
• Previous experience managing a 2532 point caseload
• Strong understanding of Medicare and compliance standards for home health
