This full-time, 13-week opportunity provides you with the chance to work autonomously in the field, managing a meaningful caseload while collaborating with a dynamic interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key responsibilities include:
- Performing thorough OASIS assessments to develop tailored patient care plans
- Administering skilled nursing care such as wound care, medication management, IV therapy, and chronic disease oversight
- Tracking patient progress, flagging changes in condition, and ensuring timely communication with the healthcare team and physicians
- Providing education to patients and families on disease, medications, and preventive strategies
- Maintaining accurate, up-to-date documentation within Home Care Home Base (HCHB)
- Coordinating with physical therapists, occupational therapists, social workers, and home health aides
- Participating in case conferences, staff meetings, and ongoing professional development
- Upholding regulatory and agency standards for all documentation and care
Qualifications:
- Active, unencumbered RN license in the state of practice
- At least one year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, and current auto insurance
- Strong assessment, critical thinking, and organizational skills
- Confidence in independent work and field-based schedule management
Preferred:
- BSN
- Experience managing a 2532 point caseload weekly
- Familiarity with Medicare/home health compliance standards
