Bring your clinical expertise and passion for patient-centered care to a vital Home Health RN role providing skilled nursing services to patients in their homes. This full-time, 13-week contract offers the chance to make a meaningful difference, independently managing a field-based caseload and supporting patients recovery journeys.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive OASIS assessments and build individualized care plans.
- Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, recognize and communicate changes in condition to physicians and care teams.
- Educate patients and families about disease processes, treatment adherence, and preventive health.
- Document all encounters and care provided accurately in Home Care Home Base (HCHB).
- Collaborate effectively with interdisciplinary team members, including therapists, social workers, and aides.
- Participate in case conferences, staff meetings, and ongoing training to maintain clinical excellence.
- Maintain compliance with all regulatory and agency standards.
Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum 1 year of recent, hands-on home health experience (field work preferred)
- Proficient in HCHB and OASIS documentation
- Reliable transportation, valid drivers license, and active auto insurance
- Strong assessment, clinical judgment, and organizational skills
- Ability to work both independently and collaboratively
- BSN preferred, prior management of a caseload of 2532 points/week is a plus
- Familiarity with Medicare and home health compliance preferred
