Bring your clinical expertise and passion for patient-centered care to a vital Home Health RN role providing skilled nursing services to patients in their homes. This full-time, 13-week contract offers the chance to make a meaningful difference, independently managing a field-based caseload and supporting patients recovery journeys.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and build individualized care plans.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognize and communicate changes in condition to physicians and care teams.
• Educate patients and families about disease processes, treatment adherence, and preventive health.
• Document all encounters and care provided accurately in Home Care Home Base (HCHB).
• Collaborate effectively with interdisciplinary team members, including therapists, social workers, and aides.
• Participate in case conferences, staff meetings, and ongoing training to maintain clinical excellence.
• Maintain compliance with all regulatory and agency standards.

Qualifications:

• Active RN license (in good standing) in the state of practice
• Minimum 1 year of recent, hands-on home health experience (field work preferred)
• Proficient in HCHB and OASIS documentation
• Reliable transportation, valid drivers license, and active auto insurance
• Strong assessment, clinical judgment, and organizational skills
• Ability to work both independently and collaboratively
• BSN preferred, prior management of a caseload of 2532 points/week is a plus
• Familiarity with Medicare and home health compliance preferred
