An opportunity is available for an experienced Registered Nurse specializing in home health care. This contract-based position calls for a compassionate professional ready to deliver high-quality, patient-centered nursing services in a field-based setting. The successful candidate will independently manage comprehensive care for patients in their homes, with a primary focus on continuity, communication, and clinical excellence.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease management.
• Monitor and evaluate patient progress, promptly reporting changes to the care team and physicians.
• Educate patients and families about conditions, medications, and ongoing care.
• Complete timely and accurate documentation within the Home Care Home Base (HCHB) system.
• Collaborate closely with physical therapists, occupational therapists, social workers, and aides to coordinate comprehensive patient care.
• Participate in staff meetings, case conferences, and necessary professional development.
• Consistently adhere to regulatory requirements and internal standards.

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent home health experience (field experience preferred).
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS.
• Strong assessment, communication, and organizational skills.
• Ability to work independently while adhering to a flexible field-based schedule.
• Reliable transportation, valid drivers license, and auto insurance.
• BSN and experience with Medicare compliance preferred.
• Previous caseload of 2532 points per week is an asset.
