Job Description
Providing skilled nursing care in patient homes, this Home Health Registered Nurse (RN) role requires clinical expertise and compassionate patient engagement. The ideal candidate is organized, self-motivated, and comfortable working independently while maintaining high standards of care and communication.
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify condition changes, and communicate promptly with care teams and physicians.
- Educate patients and families on disease management, medications, and preventive care.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary team members such as physical therapists, occupational therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and continuing education.
- Adhere to all regulatory and agency standards for documentation and care delivery.
Qualifications:
- Active RN license in good standing with the state of practice.
- At least 1 year of recent Home Health experience; field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments required.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication, time management, and organizational abilities.
- BSN preferred.
- Experience managing 2532 points per week desirable.
- Knowledge of Medicare and home health compliance standards a plus.
Schedule and Location:
- Full-time, 13-week contract.
- Monday to Friday, 8:00 AM to 5:00 PM.
- Field-based with daily travel to patient homes within the assigned territory; mileage reimbursement provided.
