Deliver exceptional patient-centered care as a Home Health Registered Nurse (RN), providing skilled nursing in patient homes. This full-time, 13-week contract role offers you the autonomy to make a meaningful impact on patient outcomes while working within a supportive and experienced multidisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop tailored care plans for each patient.
• Deliver skilled nursing care such as wound care, medication administration, IV therapy, and management of chronic diseases.
• Monitor patient progress, promptly communicate changes to physicians, and coordinate care with the interdisciplinary team.
• Educate patients and families on treatments, medications, disease management, and preventive care techniques.
• Maintain accurate, timely documentation in Home Care Home Base (HCHB).
• Work closely with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated, high-quality care.
• Engage in case conferences and ongoing professional development.

Required Qualifications:

• Active RN license in good standing in the state of practice.
• Minimum of 1 year recent experience in home health (field-based preferred).
• Proficient in Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, critical thinking, and time management skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible, independent, field-based schedule.

Preferred Qualifications:

• BSN degree.
• Experience managing a 2532 point weekly caseload.
• Familiarity with Medicare and home health compliance standards.
