Job Description

Deliver essential nursing care and make a difference in patients lives in a home-based environment. This contract opportunity calls for a skilled and compassionate Registered Nurse experienced in home health settings, providing direct, patient-focused care to individuals across varied needs.

Schedule & Hours

  • Full-time, 13-week contract
  • Monday to Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (averaging 35 patient visits daily)
  • Field-based position with daily travel to patient homes (mileage reimbursed)

Key Responsibilities

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
  • Continually evaluate patient progress, communicate changes to physicians and care teams, and ensure accountability toward health goals
  • Provide patient and family education regarding diagnoses, medication regimens, and treatment plans
  • Accurately and promptly document care using Home Care Home Base (HCHB)
  • Coordinate and collaborate with physical therapists, occupational therapists, medical social workers, and home health aides
  • Participate in case conferences, staff meetings, and ongoing professional education
  • Maintain compliance with regulatory standards and best practices in home health

Qualifications

  • Active RN license in good standing (state of practice)
  • Minimum 1 year of recent home health field experience required
  • Proficient in Home Care Home Base (HCHB) and OASIS documentation
  • Reliable transportation, valid drivers license, and current auto insurance
  • Strong assessment, critical thinking, and communication skills
  • Ability to work independently and manage a flexible schedule
  • BSN preferred
  • Experience managing point caseloads of 2532 and knowledge of Medicare/home health compliance highly valued

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
