Job Description
Deliver skilled, patient-centered nursing care to individuals in their homes as a Home Health Registered Nurse (RN) on a 13-week full-time contract. You will utilize your clinical expertise and independence to ensure continuity and quality of care across a variety of patient needs in a dynamic field-based environment.
Schedule & Assignment Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based: patient homes within assigned territory
- Caseload: 2532 points weekly (typically 35 patient visits per day)
- Daily travel required (mileage reimbursement provided)
- Documentation: Home Care Home Base (HCHB) and OASIS
Key Responsibilities:
- Perform comprehensive OASIS assessments; develop individualized care plans.
- Deliver skilled nursing care: wound care, medication administration, IV therapy, chronic disease management.
- Monitor and document patient progress, promptly communicate changes to healthcare team and physicians.
- Educate patients and families on diagnosis, treatment, medications, and preventive care.
- Ensure accurate, timely documentation in HCHB.
- Collaborate closely with therapists, medical social workers, and home health aides.
- Participate in case conferences, staff meetings, and continuing education.
- Adhere to regulatory and agency standards for all care and reporting.
Qualifications:
- Active RN license in good standing
- Minimum 1 year recent home health experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB)
- OASIS experience required
- Strong clinical assessment and critical thinking
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible schedule
Preferred:
- BSN degree
- Experience managing 2532 points per week
- Knowledge of Medicare and home health compliance
