Job Description

Deliver high-quality, compassionate nursing care in a field-based role supporting patients in their own homes. A Home Health Registered Nurse is needed for a 13-week contract focused on skilled and patient-centered care, working with a collaborative interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Expect daily travel within an assigned territory (mileage reimbursement provided)
• Caseload target: 2532 points per week (typically 35 patient visits daily)

Key Responsibilities:

• Complete comprehensive OASIS assessments; develop and manage individualized care plans.
• Deliver skilled nursing services including wound care, IV therapy, chronic disease management, and medication administration.
• Monitor patient progress, promptly communicate changes, and adjust care plans accordingly.
• Educate patients and families on disease processes, treatment, and preventive care.
• Document all care activities accurately and in a timely manner using Home Care Home Base (HCHB).
• Coordinate care with physical and occupational therapists, medical social workers, and home health aides.
• Attend relevant team meetings and maintain regulatory and clinical standards.

Required Qualifications:

• Active RN license (state of practice, in good standing)
• At least 1 year recent Home Health RN experience, with field work preferred
• Proficiency in Home Care Home Base (HCHB) and OASIS documentation required
• Strong clinical assessment, time management, and organizational skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently

Preferred:

• BSN degree
• Experience managing a 2532 point caseload per week
• Knowledge of Medicare and home health regulations
