Job Type: Full-time, 13-week contract
Schedule:
- Monday through Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct thorough OASIS assessments and create personalized care plans tailored to patient needs.
- Provide skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient progress, promptly communicating any changes to physicians and care teams.
- Educate patients and families regarding diagnosis, treatment options, medication management, and preventive strategies.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic health records.
- Coordinate care with physical therapists, occupational therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing educational opportunities.
- Ensure compliance with regulatory standards and clinical protocols.
Qualifications:
- Active RN license in good standing within the state of practice
- Minimum of 1 year recent home health nursing experience, preferably field-based
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical judgment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible field-based schedule
Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 patient points per week
- Knowledge of Medicare and home health compliance standards
