Job Description
Deliver high-quality patient care as an experienced Home Health Registered Nurse (RN). In this contract role, you'll provide skilled nursing services to patients in their homes, supporting recovery and well-being through individualized care plans and strong collaboration with interdisciplinary teams. This position is ideal for an adaptable, detail-driven professional who thrives in an independent, field-based environment and is passionate about patient education and advocacy.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload of 2532 points per week (typically 35 home visits per day)
Qualifications:
- Active RN license in good standing within the state
- At least 1 year of recent Home Health field experience
- Proficiency with Home Care Home Base (HCHB) electronic system
- Proven experience with OASIS assessments
- Superior critical thinking and clinical assessment abilities
- Valid drivers license, reliable transportation, and current auto insurance
- Strong organization, communication, and time management skills
- BSN and experience managing Medicare compliance preferred
Key Responsibilities:
- Complete OASIS assessments and develop tailored care plans
- Deliver skilled nursing services, including wound care, IV administration, medication management, and chronic disease support
- Monitor patient condition, evaluate progress, and communicate with physicians and interdisciplinary teams
- Educate patients and their families on disease processes, care routines, treatments, and preventive measures
- Timely and accurate documentation in Home Care Home Base
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides
- Participate in case conferences and ongoing training
- Adhere to federal, state, and agency standards for in-home care
