An experienced Home Health Registered Nurse is needed to deliver skilled nursing care to patients in their homes, focusing on quality, continuity, and patient-centered support. This role demands a self-motivated and organized professional who thrives working independently while maintaining high clinical standards and effective patient communication.

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Provide skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, detect changes in condition, and communicate promptly with care teams and physicians.
• Educate patients and families about disease management, treatment plans, medications, and preventive care.
• Document care accurately and timely using the Home Care Home Base (HCHB) electronic system.
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and ongoing education.
• Maintain compliance with all regulatory and agency standards.

Qualifications:

• Active RN license in the state of practice, in good standing.
• Minimum one year of recent home health experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS requirements.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Excellent communication, organizational, and time management skills.
• Ability to work independently with a flexible, field-based schedule.

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points per week caseload.
• Knowledge of Medicare and home health compliance standards.

Contract Details & Benefits:

• Full-time, 13-week contract.
• Schedule: Monday through Friday, 8:00 AM 5:00 PM.
• Typical caseload: 35 patient visits daily (2532 points per week).
