Job Description

An opportunity is available for a skilled and compassionate Registered Nurse with home health expertise to deliver high-quality patient care in a field-based environment. As a Home Health RN, you will play a critical role in supporting patients recovery and well-being in the comfort of their homes, collaborating with a multidisciplinary team and maintaining clinical excellence throughout each patient interaction.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Caseload: 2532 points per week (typically 35 patient visits per day)
  • Daily travel within assigned territory (mileage reimbursed)

Key Responsibilities:

  • Complete comprehensive patient assessments using the OASIS tool and formulate individualized care plans.
  • Deliver skilled nursing interventions, including wound care, IV therapies, medication administration, and chronic disease management.
  • Recognize changes in patient conditions, promptly communicate updates to physicians and the care team, and proactively adjust care plans as needed.
  • Provide detailed education to patients and families related to diagnoses, treatments, medications, and preventive care.
  • Accurately document all care and communication using Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides to coordinate patient care.
  • Attend case conferences and staff meetings, and participate in ongoing training as required.
  • Ensure compliance with agency and regulatory standards for patient care and documentation.

Qualifications:

  • Active RN license (state of practice), in good standing
  • At least 1 year of recent home health experience (field experience strongly preferred)
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments required
  • Strong clinical assessment, organizational, and communication skills
  • Reliable transportation, valid drivers license, and active auto insurance
  • Ability to independently manage a field-based caseload

Preferred:

  • BSN
  • Familiarity with Medicare and home health compliance
  • Experience managing a similar caseload
